I built my own and had my roommate bring back a kotatsu heater the last time he was in Japan. But I was moving furniture around the living room one day and set the couch down on the on-off switch on the power cable. Mine is a Metro 600W heater.
Despite what everyone online says, it doesn't actually need a step-down transformer. My heater was purchased in the Tokyo area, so it's rated 110V 50Hz. US power is 120V 60Hz. With the extra ten volts it can get a bit hotter than originally designed, but I've left it on all night while sleeping under it and it hasn't started a fire yet. It's so rare to be running it at full-blast anyways.

Just be very careful with how you pack stuff for the trip back. I go to Japan almost every year for work, and due to the abuse my checked luggage goes through on the way back, my bring-backs are mostly limited to cheap figures that I win from UFO catchers.
